City Care Partnership is one of the UK’s leading independent providers of specialist services for adults with learning disabilities or Autistic Spectrum Condition. With more than 20 years experience we provide bespoke and innovative assessment and care packages in a range of supported living models which include: outreach support within the person’s own tenancy or family home, 24 hour domiciliary care and residential care home support services.
We are an accredited organisation who are justifiably proud of both the achievements of the people we care for and the people who provide that support. Our highly qualified staff receive training in Positive Behaviour Support (PBS) a valued based approach which forms the foundation of service delivery and permeates all aspects of each person’s care.

City Care works closely with external organisations and local authorities. In partnership with Manchester City Council we developed the North West’s only transition service designed specifically to meet the needs of young people with Autism. A purpose built environment with integrated technology compliments PBS approaches to encourage and nurture independence as people negotiate the path from children’s to adult services.
Our aims are simple “Manage Risk, Maximise Life” an approach that sees individuals exceed not only their own expectations but also those of their families and friends. Tailored support provides individuals with a pathway to success; developing life skills, gaining self esteem and confidence. Enhanced opportunities lead to a rich and rewarding life in a supportive and safe environment.

If your child is facing transition to adult services we understand the fears and anxieties of that young person and their family. There are a lot of factors to consider and getting accurate information and support during transition is very important. We have successfully helped others through this process and whilst the challenges can seem daunting, they can also be exciting and rewarding for all involved. If you are in this position and are unsure of the way forward, we would be more than happy to discuss it with you.

Innovative Tenants Voices ensure that everyone has the chance to have their say and this is supported through Family Forums and questionnaires. Running alongside this is our Quality Assurance Manager does regular inspections, unannounced visits & audits of the services. We are registered with the Care Quality Commission and we are very proud to be fully compliant in all our areas of support.
