When most people reach adulthood, they move on from their family home and we believe that the same option should exist for people who have a learning disability. We support young people and their families through the sometimes di cult transition from Children’s to Adult Services. Following a period of assessment within our transitions services we can identify the best possible package of support and how to assist the individual to move on to the most appropriate living environment.
In July 2012 in collaboration with Manchester City Council, City Care Partnership opened the North West’s only purpose built transition service designed specifically to meet the needs of young people with Autistic Spectrum Condition.
Our dedicated Transition Service at Broom Lane (Autism Accredited) provides a range of clinical and specialist assessments, support and accommodation in a unique autism friendly environment. We work closely with families and their existing support network to facilitate person centred transition planning meetings. We help and support those individuals in exercising choice, experiencing independence and developing their skills and relationships within their community to maximise their lives as adults.
