City Care aims to provide a home for life with the option of 24 hour support within their own tenancy to people who are 16 years and older. Based on principles of Positive Behavioural Support, our specialist support services and clinical sta provide responsive person centred services to people who have a learning disability or Autism Spectrum Condition.

Before we offer a package of support to an individual, we will carry out a comprehensive assessment, identifying their requirements in terms of the type of support, the setting and the skill base and knowledge required of the team supporting them. We work in partnership with a range of housing providers to enable us to identify the right kind of accommodation. We work alongside each individual, as well as their friends, family, carers, advocates and professionals to develop a person centred package of support that will best meet the individual’s wants and needs.
