From emergency intervention support to Residential care, City Care Partnership offers a full range of services to suit individual needs. In 2013 we opened Broom Lane (pictured) , a unique combination of transition services with varied supported living accommodation.

Providing individual care to meet and exceed individual support needs
At City Care Partnership, we have always developed our services to meet the needs of individuals who would often struggle in mainstream community services. From its inception in 1993, our organisation has created a diverse range of supported living models to ensure people with a learning disability in Manchester have services designed to assist them to stay within their community of Manchester.
The need to provide individual services to people has created growth that meets needs in diverse environments. We work with young people at home, the young person who has reached adulthood and needed to become independent, the person so marginalised and damaged that no other community service provider is available to them, and those who are so vulnerable that they need registered provision. We also provide community housing to people who we promised a home for life. All require very different types of provision, care and support.
People with learning disabilities and their families sometimes need that extra bit of support. Our belief that this support should be within their own community has resulted in the development of our Outreach Support Team.

If your child is facing transition to adult services we understand the fears and anxieties of that young person and their family. There are a lot of factors to consider and getting accurate information and support during transition is very important. We have successfully helped others through this process and whilst the challenges can seem daunting, they can also be exciting and rewarding for all involved. If you are in this position and are unsure of the way forward, we would be more than happy to discuss it with you.

Innovative Tenants Voices ensure that everyone has the chance to have their say and this is supported through Family Forums and questionnaires. Running alongside this is our Quality Assurance Manager does regular inspections, unannounced visits & audits of the services. We are registered with the Care Quality Commission and we are very proud to be fully compliant in all our areas of support.
